Michael Rosenblum, MD Joins Masshealth Patient-Centered Medical Home Initiative Steering Committee

Dr. Michael Rosenblum, Associate Program Director, Internal Medicine Residency, was invited to join the steering committee of the PCMHI—a statewide initiative to create a more efficient and effective model of health care delivery. PCMHI, started in 2009, seeks to address care fragmentation, challenges associated with the increasing prevalence of chronic disease, and a potential shortage of primary care physicians.


In a patient-centered medical home, the primary care provider team coordinates all of a patient's health needs, including management of chronic conditions, visits to specialists, hospital admissions, and reminders about check-ups and tests.


Health New England is one of the participating payers, and Baystate's High Street Health Center-Adult Medicine, High Street Health Center-Pediatric Medicine, and Mason Square Neighborhood Health Center are among the 46 sites chosen from the 84 primary care sites that applied to participate in PCMHI.
