In support of its educational mission, Baystate Health System is pleased to provide clinical experiences for Advanced Practice students (NP, PA) in a variety of topics.

To be considered for clinicals at one of our facilities, please complete the application for a student clinical experience and submit to the Advanced Practitioners office. 

If you are requesting more than one rotation, you will need to submit a separate form for each request. Please submit your application at least 30 days before your desired rotation date.

Once a rotation is approved and scheduled, several requirements need to be submitted
including:

• Background Release form (waived if current BH employee)
• Health Record (includes flu declination)
• Security ID form
• Online Training lessons (waived if current BH employee)

All requirements must be completed prior to start of your clinical rotation.  Additionally, an affiliation agreement must be in place between your school and Baystate Health System before you begin.
