Osteopathic students electing to participate in a 4th year elective(s) will need to submit the following to Baystate Medical Center:

A copy of your school’s elective request form or a letter specifying what elective(s) you are applying for

"Letter of Good Standing" from the Dean

Copy of current transcript

Malpractice face sheet with coverage of at least $1,000,000 per occurrence and a $3,000,000 aggregate

Immunization record
-
Proof of health insurance

$150.00 application fee per rotation for up to 3 rotations. Checks should be made payable to Baystate Medical Center

To be considered for the Emergency Medicine or Neurology electives at Baystate Medical Center, applicants must also submit a letter of intent.
All materials should be mailed together to the address below. Missing items will delay the approval process.

Once all application materials have been received, this packet of information will be forwarded to the appropriate Elective Director for review.
The approval process will take 3 to 4 weeks and the student will receive an email notification of the final approval. The student must be properly registered at least one month prior to the start of the elective which includes receipt of the processing fee.
