The Baystate Pharmacy Department participates in the American Society of Health-System Pharmacists (ASHP) Resident Matching Program. Visit www.ASHP.com for more information. The BMC PGY-1 residency program has 3 positions available for 2010-2011.

Application Package

  -  Transcript from Pharmacy School

  -  Three (3) Letters of Reference

  -  Letter of Intent

  -  Curriculum Vitae

  -  Application for PGY1 Pharmacy Practice Residency 2010 - 2011 (Please print and complete)

  -  Applicant’s Questionnaire for Pharmacy Residents (Please print and complete)

Requirements for Acceptance

• Eligibility for licensure in Massachusetts

• PharmD degree or equivalent experience
