Application Process

The Baystate PGY-1 Pharmacy Department participates in the American Society of Health-System Pharmacists (ASHP) Resident Matching Program. Visit www.ASHP.com for more information. The BMC PGY-1 residency program has 4 positions available for 2011-2012.

Online Application

Online applications open December 1, 2010 and close January 7, 2011. A link to the application will be provided here.

Application Package

In addition to the online application, you must provide the following:

• Pharmacy School Transcript (sent directly from your school)

• 3 Letters of Reference (sent directly from each reference)

• Letter of Intent

• Curriculum Vitae

• Applicant’s Questionnaire for Pharmacy Residents (Please print and complete)

The Application Package materials may be either:

Requirements for Acceptance

• Eligibility for licensure in Massachusetts

• PharmD degree or equivalent experience

• Online application completed by the deadline

• Completed application package submitted by the deadline
