Applying to the PGY-1 Pharmacy Residency

Application Process

The Baystate PGY-1 Pharmacy Department participates in the American Society of Health-System Pharmacists (ASHP) Resident Matching Program. Visit www.ASHP.com for more information. The BMC PGY-1 residency program has 5 positions available for 2012-2013.

Online Application

Online applications open December 1, 2011 and close January 1, 2012. 

Application Package

In addition to the online application, you must provide the following:

• Pharmacy School Transcript (sent directly from your school)
• 3 Letters of Reference (sent directly from each reference)
• Letter of Intent
• Curriculum Vitae
• Applicant’s Questionnaire for Pharmacy Residents (Please print and complete)

Submitting the Application Materials

1. Complete an online application to the program: (a link will be posted here); online applications will be accepted between December 1, 2011 and January 1, 2012.
2. E-mail a completed application package (Curriculum Vitae, Letter of Intent and Applicant Questionnaire) to Aaron.Michelucci@baystatehealth.org by January 1, 2012 at 11:59 pm
3. Ensure 3 letters of reference and all appropriate transcripts are mailed to:

Requirements for Acceptance

• Eligibility for licensure in Massachusetts
• PharmD degree or equivalent experience
• Online application completed by the deadline
• Completed application package submitted by the deadline
