Baystate Children's Hospital "What's My Name?" Contest             Official Rules

• This contest is open to all children, teens, and young adults ages 5 through 21. 
• One entry per person. 
• Entry forms must be completed in their entirety. 
• All entries become the property of Baystate Health/Baystate Children’s Hospital. 
• There will be one grand prize winner selected by a panel of judges. Ten additional entries will be selected for a random prize drawing. The decision of the judges is final.
• The Grand Prize Winner’s name and photo will appear in the next issue of Baystate Health magazine and on the Baystate Children’s Hospital website. 
• The Grand Prize Winner will receive a prize package featuring a $100 gift card to their choice of Dick’s Sporting Goods, Barnes & Noble Bookstore, or Toys ‘R’ Us.
• Entries will be accepted electronically using the online entry form, or the entry form may be copied and mailed to:
