Geriatric Outpatient Evaluation Clinics


These clinics offer a range of services for the elderly in the community. Patients with a history of falls, memory deficits, inability to care for themselves, incontinence, or other problems common to seniors can be seen and receive a "second opinion" and expert advice for themselves, their families, and their primary care providers. The clinic is staffed by a multidisciplinary team consisting of a physician, nurse, and social worker with special training in geriatrics. The team (and other health care providers as needed) can provide consultation and assist with the evaluation and care of the complex older patients who are acutely ill.

 

The clinics are held weekly at the High Street Health Center Clinic on the South Campus of Baystate Medical Center, 140 High Street, Springfield, MA.

HIV Program


This program is located within HSHC at 140 High Street, Springfield, MA.  The program consists of a multicultural, bilingual team of health care professionals that support people with HIV disease and those who are at risk for HIV disease.  Under the direction of Lauren Meade, MD, director of HIV Coordinated Services, the clinical team consists of a case manager, nurse, clinical pharmacologist, and physicians in training dedicated to HIV.  The team also offers patient financial, emotional, and educational support. 

 

The team identifies patients who are candidates for powerful drug therapy against HIV disease and maintains close contact with the patient during the process of treatment.  Information is also available to assist with obtaining HIV medication for procedures that have not yet received FDA approval for patients who choose to trail experimental protocols. 

 

For patients who are HIV negative, teaching is available to reduce the risk of contracting HIV if the patient is sexually active or using intravenous drugs.

Transgender Service

 

Baystate's transgender service provides multidisciplinary primary medical care and supportive services to patients seeking hormonal and/or surgical reassignment of gender. We offer a holistic approach to individual health, working closely with local and regional therapists and specialists as needed.

 

We strive to provide care that is sensitive to each individual’s needs, regardless of where they see themselves on the transgender spectrum. Baystate provides supportive and sensitive care regardless of gender identity, gender expression, sexual orientation, race/ethnicity, religion, or socio-economic status.

 

Baystate has developed services based on the Informed Consent model of care, meaning that we work with our patients to ensure that the risks and benefits of gender-related services are fully understood at all times. Gender-related therapy is an encouraged--but not required--part of treatment for most patients.
