Baystate Visting Nurse Association and Hospice

Homecare continues to make strides in terms of standardized practices and expanding the scope of services it provides.  Now regarded as true partners in care, homecare services are critical to ensuring patients get the right services at the right time to help them recover from acute illnesses as soon as possible, and to manage chronic diseases at home.  

 

Baystate Visiting Nurse Association and Hospice (BVNAH) and Baystate Home Infusion and Respiratory Services (BHI&RS) are working even closer together to provide a coordinated model of care.  As part of the State Action on Avoidable Rehospitalizations (STAAR) project, both programs have been key to our success in decreasing heart failure rehospitalizations and the overall rehospitalization rate.
