Accidental falls are the most common patient safety incident in the hospital causing serious injury or death, and are especially common in older patients. Because of this, Baystate Health's Patient Care Services initiated the Fall Prevention Program, which has been very helpful in preventing patient falls and injury since it began.
All patients at Baystate Health are assessed for their risk of falls when they are admitted to the hospital. Certain populations, such as the elderly, post-operative patients and patients receiving certain medications, are at higher risk of falls. Based on the risk score that each patient is assigned, the care team gives preventive care tailored to each patient.
Things such as changing the time a medication is given, having bed alarms on high-risk patient beds, and hourly patient rounding have helped to keep the falls rates low at all three Baystate Hospitals.
