All Tests and Procedures are done by Appointment Only.  Call the lab at 794-1958 to schedule an appointment.  Driving directions....

IUI:  During the week we ask anyone using our collection rooms to arrive at 7:00am and anyone collecting the sample at home to drop the sample off at 7:15am.  On weekends and holidays, everyone is asked to arrive at the lab at 7am.

Semen Analysis:   Appointments are made at 10:00am, 10:30am, 2:00pm & 2:30pm.  Semen Analyses are not done on weekends or holidays. 

Semen Freeze:   Appointments are made at 10:00am, 10:30am & 2:00pm.  Semen Freezes are not done on weekends or holidays. 

Practice Preps:  Appointments are made at 10:00am & 10:30am.  Practice Preps are not done on weekends or holidays.

After making an appointment, call registration at 794-9600 to give your health insurance information.  Registering also insures that your results are properly reported to your doctor.
