Medical Home Care Coordination Program

 

MEDICAL HOME PROGRAM

A Medical Home is a process of care offered by primary care providers in partnership with families of children with special health care needs (CSHCN).

Children and families are recognized, welcomed and supported by their community based medical homes.

A Medical Home is not a building, house, or hospital, but rather an approach to providing health care services in a high- quality and cost effective manner.

 

CARE COORDINATION PROGRAM

The Care Coordination Program is part of the Division of Perinatal, Early Childhood and Special Health Needs within the Massachusetts Department of Public Health. If your child has a special health care need; for example, a chronic illness, a disabling condition, or a frequent need for medical technology, it can be difficult and frustrating to get the information, care, and services needed. This program provides care coordinators whose job it is to help you navigate the system to obtain the programs and services available

The Care Coordinators can help you:

 

  • Identify the needs of your child and family
  • Understand the full range of available public benefits
  • Identify community resources to assist you
  • Gain access to specific programs and services
  • Become a more effective advocate
  • Connect with other families who face similar challenges

PARENT RESOURCE CENTERS

Parent resource centers offer a wide variety of information relating to caring for CSHCN. They offer lending libraries, free brochures, and postings of upcoming community events.

Parent resource centers are located at 140 High Street and also at 3300 Main Street.

 

 

MEDICAL HOME MEETINGS

Medical Home network meetings are held monthly from September to June. The meetings are designed to bring together providers, community organizations and families around health care issues for CSHCN in the Baystate Children’s Hospital System.
