• Committed to patient centered care, we actively partner with our patients and their families to address their needs and define their desired outcomes. With the patient/family as active participants and the focus of care, we act as advocates and coordinators of care as we collaborate with other members of the team to meet defined outcomes.
  
• We acknowledge patients as multi-faceted, unique and dynamic beings. We also recognize the family and community as integral to care. We value the multicultural dimensions of our patients, their families and community. Our commitment extends across the life span and addresses health promotion, maintenance, and disease prevention.
